RE : STUDENT EVALUATION

The Office of the Registrar will now accept an application for evaluation for the Second Semester of A.Y. 2020-2021. The schedule of filing of evaluation is on April 12-24, 2021.

EVALUATION PROCEDURES

I. ONLINE PROCEDURES

1. Visit our Official BCU Facebook account;

2. Download the PDF Request Form;

3. Fill out all the necessary information required therein;

4. Send the scanned complete request form to bcuregoffice2016@gmail.com;

5. After we have received and verified your application, a notification will be sent to you as to when will your evaluation form be available.

II. ON-CAMPUS PROCEDURES

Students are required to proceed for triage and fill out the necessary forms and observe strict physical distancing measures.

1. Fill out the request form;

2. Afterwards, submit the form to your respective window.

NOTE:

1. Application for evaluation submitted on April 12-24, 2021 will be free of charge.

2. Please be informed that transferee students should have already submitted their OTR before applying for evaluation. Otherwise, your application will not be processed.

3. Application sent after the scheduled dates will already be subject to necessary fees.

4. Application sent through our email address after the scheduled dates will no longer be entertained.
